The Information Sharing Request Application is a form for GuideWell and Florida Blue employees to gain permission to share certain types of information to approved entities.*

PROBLEM AND APPROACH
Since it is such a robust form, my goal was to make the process as smooth, fast, clear, and painless as possible — from filling it out, to quickly comprehending the content, to understanding the value our business provides, and so on.
There are lots of things we can do in UX to help people get through long forms faster, with more clarity, and fewer if any painpoints. These refinements helped reduce user frustration in working with our team, and encouraged their continued cooperation with our needs. 
Since we were working with developer limitations, I wasn't able to do an overhaul, but was able to influence things like spacing, language clarity, button refinements, more appropriate form elements, content logistics like categorization and order, content consolidation and reduced redundancies, and general form building best practices.
Results
These improvements have resulted in markedly speedier completion rates and fewer inquiries to our team, all due to visual and verbiage clarity and reduced painpoints.

The redesign
